Send completed form to your regional office. We’ll review your project plans to identify the risks to fish and fish habitat associated with your project. During our review, we’ll determine: * whether your project will impact an aquatic species at risk * if your activity can also result in the death of fish and the harmful alteration, disruption or destruction of fish habitat * if your project will need authorization under the Fisheries Act
